Often times you can download one from your states dmv website. Here is an example from TN.

Bill of Sale TN pdf

In my county you either need a title signed over on the back or you have to file for lost title if there is none.

Calif has one on their DMV web site...

Something very simple that just states the facts typed out in word and signed by concerned parties will do...

GOOGLE search for "bill of sale form" turns up hundreds of possibilities...

I usually just hand write, with the following information, vehicle description(year, make, model) vin #(serial number)amount paid, sellers name and address and signature and buyers name, address and signature. I also include the date the transaction took place, If I have access to a copier I will make a copy for my records. I think in the last couple years our state has incorporated a bill of sale onto the bottom of our titles.
If I could shut my brain off, I could save myself alot of time, money and effort.
