Not sure how it is in the US but here in the UK usb dongles are widely used but reception out of cities can be very poor.

Mobile phone (such as an android or iphone) acting as a wifi hotspot and using your phones data allowance seems to be the most cost effective way here. (Turns your phone into a wireless access point for your laptop/ipad etc)

Over here a dongle alone on pay-as-you-go (prepaid) is about £10 a month for 1-3gb

Most of this doesn't answer your question some of it does My brother lives in an area where there isn't any DSL service
he has a AT&T 4G mifi card normal cost is $60 a month
He worked for phone company and gets a huge discount house phone, cell phone & mifi card $80 a month.

when camping and sometimes when I'm in a motel/hotel that has weak wifi signal I use my ALFA mifi dongle
it comes with a small 6 inch antenna but I also have a 18 inch yagi beam antenna when the beam antenna
is hooked up and running I can pull in signals from miles away.
from my house here at the lake it is about 5 miles using the road to town and McDonalds but about 2 miles line of sight
I am able to use their wifi from my house I don't because it is to slow, but the ALFA & beam comes in handy when camping
for weak signals but out in the boondocks where no one has wifi then I can tether my phone I've only done this once.

Nope. I take my tech with me. A campfire and an ebook is a hard combination to beat.

It's stress I try to get away from, not technology.
The more stuff I take along, the more time I spend taking care of my stuff!

I have used Verizon Mifi exclussively for 5 years because of the portability factor. Yes, it consts abouit $15 more a month compared to cable internet. The portability fits into my lifestyle extremely well.

Works well for me for general use. Will not work for streaming movies or gaming. Essentially if I have cell phone reception I have internet reception. Can be extremely slow in some locations. At this point plan on using Mifi indefinetely.

May not answer your intended question of pay as you use internet.
